Milkweed Sap: Skin Contact, Eye Exposure, and Why It Needs to Be Taken Seriously

Milkweed sap contains cardiac glycosides that can cause real skin irritation -- and, in the eye, a genuine medical emergency that is frequently misdiagnosed as simple conjunctivitis. What the exposure actually does, and what to do about it.

Milkweed is a beloved garden plant -- essential to monarch butterflies -- but its milky sap is not harmless. It contains cardiac glycosides that can irritate skin on contact and, if it reaches the eye, cause a real and sometimes vision-threatening injury that emergency physicians have flagged as commonly missed on first evaluation.

A different mechanism entirely: direct cellular toxicity

Milkweed's white latex sap -- found throughout the plant but most concentrated in the sap itself -- contains cardenolides, a class of cardiac glycosides chemically related to digitalis (the source of the heart medication digoxin). These compounds work by inhibiting the sodium-potassium pump (Na+/K+-ATPase) that cells rely on to function. This is not an allergic reaction like poison ivy and not a light-activated reaction like phytophotodermatitis -- it is a direct toxic effect on the cells it touches, which is part of why eye exposure is taken so seriously.

Skin contact: real irritation, a known gardening hazard

On skin, milkweed sap can cause noticeable irritant contact dermatitis -- redness, irritation, and in some cases more significant reaction -- and is a recognized hazard for gardeners who handle the plant regularly, particularly when cutting or pruning stems where sap flows freely. Some sources note that residue can remain on skin even after it appears to dry, and that sweat can reactivate it.

Eye exposure is a genuine medical emergency

This is where milkweed sap differs meaningfully from most other plant-contact reactions in this cluster. Direct ocular exposure can disrupt the eye's corneal endothelial pump function, causing corneal swelling (edema) and a real, sometimes significant, loss of vision within hours to a couple of days. A 2026 case report described a contact-lens wearer whose vision was reduced to motion detection after gardening; she was initially misdiagnosed with simple conjunctivitis and discharged, then returned with worsening symptoms before the plant exposure was identified and she was treated with corticosteroids. Emergency physicians have specifically flagged this condition as one that is easy to miss without a clear exposure history.

Do not eat any part of the plant, despite folk claims otherwise

Some online sources describe cooking or preparing milkweed pods, shoots, or an "elixir," sometimes claiming that cooking removes the cardiac glycosides because they are water-soluble. Poison control organizations state plainly that this is not established and that eating milkweed, cooked or not, is not recommended. Real poisoning cases exist, including a documented case of cardiac symptoms, a slowed heart rate, and a detectable digoxin cross-reactivity level in a man who ate fried milkweed pods from an online recipe. Symptoms of ingestion can include nausea, vomiting, diarrhea, weakness, and heart rhythm changes.

Practical prevention for gardeners

Wear gloves when cutting, pruning, or handling milkweed, and wash hands and any exposed skin promptly afterward. Avoid touching your face or eyes while working with the plant, and be especially cautious around children, who may not connect a stinging or blurry eye to something they touched earlier.
