What the evidence says

A study of runners in the Western States Endurance Run, a 161-kilometer ultra-endurance event with an electrolyte-supplement usage rate over 90% among participants, found little evidence that electrolyte supplementation reliably prevented hyponatremia -- and identified drinking beyond one's thirst, not insufficient sodium intake, as the more consistent risk factor.

Hyponatremia (low blood sodium) is a genuinely dangerous condition, most commonly caused by drinking more fluid than the body needs rather than by insufficient electrolyte intake -- following thirst as a guide, rather than a fixed hydration schedule, is a more evidence-aligned approach for most people during moderate exercise. Anyone with kidney disease, heart failure, or a condition affecting fluid or electrolyte balance should get personalized hydration and electrolyte guidance from a clinician rather than relying on general sports-nutrition advice, since both too much and too little of a given electrolyte can be genuinely dangerous in those conditions specifically. Severe symptoms during or after exercise -- confusion, seizure, severe headache, or vomiting -- warrant emergency medical attention rather than a supplement.
